Health Monitoring |
|
The Health Office strives to help every student overcome any medical or physical obstacle that compromises his or her academic success at Hubble. Towards that goal, chronic illnesses, hospitalizations, attendance and injuries are monitored. Medications taken at school may be evaluated for effectiveness. Communication between student, parent, and school staff is encouraged. The school nurse will work with students and parents to minimize school absences. Students who are not able to attend school for over one week due to a medical concern, or have excused absences for more than 10% of the attendance days, will be contacted. The school nurse may request an updated school physical to document any significant changes in health. Illinois Department of Health School Physical form (pdf format) is available. Health Services |
|
The Health Office assists students with immediate health problems and encourages them to develop good health. The school nurses work closely with all members of the student personal service team, which includes counselors, psychologists, administrators, and social workers. |
|
A student may be sent to the Health Office on a teacher’s pass in case of sudden illness or injury. If the nurse decides a student is to be sent home, a parent, or another authorized emergency contact must arrange transportation. |
|
A student who is ill and absent from school due to a communicable disease must see the nurse before returning to classes. Some communicable diseases require a doctor’s release when the student returns to school. |

Vision and Hearing |
|
Screenings are conducted annually by the DuPage County Health Department in the fall for all students in special education, foster care, and new students. All 8 th grade students have their vision screened. If you have any concerns regarding your student’s vision or hearing, please call (630) 682-2172 or e-mail Kbarry@CUSD200.org. |

PE Excuses and Modifications |
|
All students are required to participate in physical educations classes unless there is medical documentation provided by a physician stating otherwise. A parent’s note is accepted if a student is out of PE for three days or less. If a student is unable to participate for more than three days, a medical excuse from a physician stating the nature, limitation, and duration of the condition is required. All efforts will be made to provide Modified PE when recommended. A Modified PE form is available. |

Medications in School |
|
Prescription and non-prescription medications will be given according to the District 200 Medication Policy. This policy states that students are not allowed to carry any medication during the school day. With proper authorization, inhalers are the only medications permitted to be carried at the middle school level. The Health Office staff will administer only medications necessary to maintain the student during school hours, with signed authorization from the student’s physician and parent. Medication must be brought to the Health Office by a parent or guardian in the original, appropriately labeled pharmaceutical container. Medication authorization must be renewed annually and whenever there is a change in the prescription. An Administration of Medication in School form is available. To aid in communication with a doctor’s office, an Authorization to Release/Exchange Information and Records form is available. |
